Make a Referral

If you would like to refer a youth to AFC Mentoring, please contact AFC Mentoring's Programming staff at (617) 224-1301.

All youth must meet the following requirements to become a mentee with AFC Mentoring:

  1. Have personal experiences in foster care, adoption, kinship care, residential care, and/or other out-of-home placements.
  2. Be at least 7 years old or older, and be willing to meet for at least 8 hours a month for one year.
  3. Live in the Boston Metro area.
  4. Attend two 2 hour mentee training sessions to prepare for the mentoring match.

Please print and complete the mentee application and either mail or fax it (617-451-1025) to our office. NOTE: The application cannot be electronically sent to us.
